Phyllis D. Horwitz nee Lebow, 87 passed away after a brief illness, October 21, 2016. Mrs. Horwitz is survived by her son Craig (Joy) Horwitz and Karen Horwitz and her grandchildren Daniel and Jennifer. Service Wednesday, 1:00 pm at Chicago Jewish Funerals, 8851 Skokie Blvd., (at Niles Center Road) Skokie. In lieu of flowers, a donation to any medical charity of your choice is appreciated. Dear Karen and Craig,

My mother and I are sorry for your loss. We loved Phillis and will miss her. It was wonderful that we could spend time with her when she visited LA a few years ago. She was often the glue that held parts of the family together. When my husband and I were in Chicago she hosted a family dinner that allowed us to visit will all. We send our condolences.
